The medical certificate and tests for a Dominica citizenship application

Every Dominica citizenship applicant completes Form D3, the medical questionnaire and certificate, with a licensed doctor. Blood and urine tests are needed, and an HIV test for all applicants aged 12 and over. The results must not be more than three months old at submission. This guide explains the order and the practical points.

The medical part of the application is simple, but its timing matters. If the tests are done too early, they may be out of date by the time the application is submitted. The information in this guide comes from the official pages of the Citizenship by Investment Unit of Dominica (CBIU), checked on 24 October 2026.

What is needed

According to the CBIU's required documents and "How to apply" pages:

  • Form D3, the medical questionnaire and certificate, completed by a licensed doctor;
  • routine blood and urine test results;
  • HIV test results for all applicants aged 12 and over.

The three-month rule

According to the CBIU's "How to apply" page, the medical certificate and tests must not be dated more than three months before submission.

Suggested order

  • First prepare the time-consuming documents: police clearance certificates, source of funds documents, translations and certifications.
  • When most of the application is ready, set an approximate submission date with the Authorised Agent.
  • Have the tests and examination done at a suitable time before that date, so they stay within three months if submission slips a little.

At the doctor's

  • Take Form D3 with you and ask the doctor to complete every part fully, legibly and with stamp and signature.
  • The name and date of birth on the form and the test reports must exactly match the passport.
  • Get the test reports on the laboratory's letterhead, with date and signature.
  • Documents not in English need translation, notarisation and legalisation.

For children

  • The HIV test is needed from age 12.
  • Minors' forms are signed by both parents or the legal guardian.

Confidentiality

Medical information is among the most sensitive parts of the application. Send it only through the secure channel the Authorised Agent names. In the Royal Golden portal, each family member's documents are uploaded separately and only you and authorised staff see them.
